Transcript Requests

Official transcripts provide a record of the courses a student has taken, the grades earned and the credits awarded at Brentwood High School. Transcripts are often required for college applications, scholarships, employment and other postsecondary opportunities. Our counseling team works closely with students and families to ensure transcripts are requested and submitted accurately and on time.

Current Students

All current students must sign the FERPA release in Scoir before transcripts can be sent.

Students using the Common Application

Click on the “My Colleges” tab in your Common Application account.
Sign the FERPA release.
List Emily Diaz as your counselor in the “recommendation” section:
- Email: ediaz@brentwoodmoschools.org
Mrs. Diaz will receive an email prompting her to upload your transcript directly into your Common Application account.

Students not using the Common Application

Log into your Scoir account.
Follow all steps outlined in Scoir’s “For Students: Requesting a Transcript” guide.

Important: Please make all transcript requests at least one week before application deadlines. Transcripts will not be sent when school is not in session.

Alumni and Former Students

Students who have graduated from or withdrawn from Brentwood High School may request transcripts and academic records by completing the FERPA Release Form. The completed form may be delivered, mailed or emailed to:

Requests are processed Monday through Friday, 8 a.m. to 3 p.m.

If you are unable to download the FERPA Release Form, you may send a written request to the address above that includes:

Name as it appears on school records
Date of birth
School last attended or school of graduation
Year of graduation or year last attended
Address where the transcript or records should be mailed
Phone number where you may be contacted
Signature of the individual making the request

Brentwood High School Diplomas

If you have lost or misplaced your diploma, we are unable to issue a replacement diploma. However, a graduation verification letter can be provided at no cost as a substitute. The letter will include your name and date of birth, confirm that you completed all academic courses and credit requirements to earn a Brentwood High School diploma, and list your graduation date.

If you never received your original diploma, please contact the Brentwood High School counseling office.
